Residency Verification 2019-2020

 

IN-PERSON RESIDENCY VERIFICATION WILL TAKE PLACE ON AUGUST 14 FROM 10:00 A.M. TO 7:00 P.M. AND AUGUST 15 FROM 7:00 A.M. TO 4:00 P.M. AT EPCHS

Dear Parents/Guardians:

You are receiving this correspondence to explain the required steps in our 2019-20 Registration Process.  According to our records, you did not complete the Residency Verification process by May 31, 2019.  Therefore, the following steps need to be completed in person to maintain your student’s enrollment.  If your student no longer plans to attend Evergreen Park Community High School, please contact the registrar at 708/398-1275.

The parent or legal guardian must attend one of the following:

 

Required Onsite Registration Sessions

Wednesday, August 14, 2019, from 10:00 a.m. – 7:00 p.m.

Thursday, August 15, 2019, from 7:00 a.m. – 4:00 p.m.

 

During the Required Onsite Registration session, families will have several steps to complete.  Please come prepared to avoid any further delay.  The steps include: 

 

  1. Residency Verification

Families will need to prove residency at the first station during the Required Onsite Registration session on Wednesday, August 14 or Thursday, August 15.  Please review the required residency documentation prior to the session.  Additionally, complete the Residency Verification Form prior to attending and bring the completed form, along with copies of the required documentation.  Again, this is the first step in the process and no other steps can be processed until your residency has been approved on August 14 or August 15, 2019.  DO NOT send documentation in advance.  The opportunity to have documents submitted in advance ended on May 31, 2019.  Bring copies of the documentation with you.  Please direct any questions to sdorgan@evergreenpark.org.

 

  1. Skyward Registration Verification

Families will update their contact information at the second station during the Required Onsite Registration session on Wednesday, August 14 or Thursday, August 15, 2019.

 

  1. Fee Arrangement

Families will have the following three options to make fee payment arrangements at the third station during the Required Onsite Registration session on Wednesday, August 14 or Thursday, August 15, 2019:

 

  1. Payments – Payment in Full

Families who are paying student fees in full can pay online or can pay with cash/credit card/check during one of the Required Onsite Registration sessions on Wednesday, August 14 or Thursday, August 15, 2019.  Please direct any questions to Kyle Nine, knine@evergreenpark.org or 708-398-1286.  

 

  1. Payments – Payment Plan

Families who wish to pay student fees on our payment plan must sign the payment plan agreement and make a payment during one of the Required Onsite Registration sessions on Wednesday, August 14 or Thursday, August 15, 2019.   Please direct any questions to Kyle Nine, knine@evergreenpark.org or 708-398-1286.

 

  1. Payments – Fee Waivers

Families who wish to apply for a fee waiver must do so in person during one of the Required Onsite Registration sessions on Wednesday, August 14 or Thursday, August 15, 2019.  The Waiver Application form is available on our website, www.evergreenpark.org/parents/2019-20-registration-information, and in the Business Office.  All documentation (including 2018 tax returns and household income documentation) must be complete in order for a waiver to be considered.  Please direct any questions to Kyle Nine, knine@evergreenpark.org or 708-398-1286.

 

Free Lunch

Families who wish to apply for free lunch must do so in person during one of the Required Onsite Registration sessions on Wednesday, August 14 or Thursday, August 15, 2019.  The free lunch application is available on our website, www.evergreenpark.org/parents/2019-20-registration-information, and in the Business Office.  Families who qualify for free lunch do not automatically qualify for a few waiver.  Please see Payments - Fee Waivers for more information.   Please direct any questions to Kyle Nine, knine@evergreenpark.org or 708-398-1286.

 

Student Schedule

Once all three steps of the Required Onsite Registration Session are completed, you will receive your student’s schedule.

FAQ:

I have a complex guardianship situation or residency matter that I wish to talk to someone in person, what should I do?

Contact Serene Dorgan at 708/398-1275 or sdorgan@evergreenpark.org to schedule an appointment or visit her on the assistance days on Tuesday, April 16, Wednesday, May 8, or Thursday, May 30, from 4:00-8:00 p.m. in the school cafeteria.

 

What is the deadline to have the residency documentation submitted?

After May 31, 2019, failure to submit residency documentation can delay your registration for the 2019-20 school year, and require attending at least one parent/guardian on campus residency/registration session on Wednesday, August 14 or Thursday, August 15, 2019.

 

My family usually qualifies for a fee waiver and/or free lunch, do I need to complete the residency verification process?

Yes.  Every family needs to complete the residency verification process on an annual basis.  The residency verification process is independent of the fee waiver and free lunch application process. 

 

I am the parent/legal guardian but I am residing with another family within the district.  What paperwork is needed?

An Affidavit of Residency must be completed and can be found on page 3 of the Residency Verification Form.  You can also obtain a copy at the security desk located inside our main entrance.  All three pages must be completed.  The property owner/lessee on the Affidavit of Residency is required to provide proof of residency and submit copies of the following:  current driver’s license or State ID, mortgage statement, homeowners insurance, or lease, one current utility bill.  The parent/legal guardian on the Affidavit of Residency must submit copies of the following:  current driver’s license or State ID AND copies of two of the following:  W-2, pay stub, bank statement, Public Aid, Medicaid, vehicle insurance, vehicle registration or other acceptable proof of living at the address used on the notarized affidavit.  If you are utilizing the Affidavit of Residency, you also may be subject to random residency checks by our police liaison, at which time you must show proof that the student and legal parent/guardian reside at said address.  No Exceptions!
