Step 1:  Annual Residency Verification (This process must be completed by May 31, 2019.)

EPCHS will collect residency documentation beginning April 1, 2019 for the 2019-20 school year.  Families are encouraged to have their annual residency documentation submitted by

May 31, 2019.  Families who complete residency verification by May 31, 2019 will receive a $25.00 fee discount per student.    

A family only needs to complete the residency process once each year.  If you enrolled an incoming freshman in January/February of this year, you have already complied with the residency verification process for not only your freshman, but for siblings who currently attend EPCHS and Step 1 is completed and you will receive the $25.00 fee discount for each student. 

Families must print the Residency Verification Form from the website or pick up a copy from the security desk, and return it along with copies of all the required paperwork.  Although families are encouraged to submit their documentation electronically to residency@evergreenpark.org, families do have the option of submitting the required residency documentation via fax (708/424-3045 or 708/424-7497), US Mail, or by dropping off copies of the required paperwork at the school’s security desk, located inside our main entrance.  The school will not make copies.  Additionally, staff members are not available during the school day without an appointment.  Residency questions and requests for appointments should be directed to Serene Dorgan at 708/398-1370 or residency@evergreenpark.org.

Families needing assistance with residency documentation can visit campus during the following assistance days on Tuesday, April 16, Wednesday, May 8, and Thursday, May 30, 2019, from 4:00–8:00 p.m., in the Leadership Room, located in the school cafeteria.

Step 2:  Skyward Verification and Fee Payments/Waivers (This process must be completed by July 31, 2019.) More information will be available on our website and emailed home after May 31, 2019.
 

Beginning Monday, July 8, 2019, families who have completed their residency verification by May 31, 2019, will need to log in to Skyward to verify their Skyward information and make a payment online, mail payment, or drop off payment to the Business Office no later than Wednesday, July 31, 2019.  Families who make a minimum payment of $100.00 per student will receive an additional $25.00 fee discount for each student.   

Families who have completed their residency verification by May 31, 2019, and who need assistance with logging on to Skyward to verify their Skyward information or if they need assistance making online payments or need to apply for a fee waiver must attend one of the following Wednesday fee payment sessions:  July 10, July 17, July 24, or July 31, 2019, from 7:00–11:30 a.m. and 1:00-5:00 p.m., in Room 102.  Families who make a minimum payment of $100.00 per student will receive an additional $25.00 fee discount for each student.

FAQ – Frequently Asked Questions

 

I have completed the residency verification process for the past three years and I haven’t moved in twenty years, do I have to do it again this year?

 

Yes.  Every family needs to complete the residency verification process on an annual basis.  However, if you enrolled an incoming freshman in January/February of this year, you have already complied with the residency verification process for not only your freshman, but for siblings who currently attend EPCHS.

 

I have limited access to the Internet and/or have limited computer skills, how can I submit my residency documents?

 

Download the Residency Verification Form from the website or pick up a copy from the security desk located at our main entrance and submit copies of all the required documentation via fax, mail, or drop-off.

 

I have a complex guardianship situation or residency matter that I wish to talk to someone in person, what should I do?

 

Contact Serene Dorgan at 708/398-1275 or sdorgan@evergreenpark.org to schedule an appointment or visit her on the assistance days on Tuesday, April 16, Wednesday, May 8, or Thursday, May 30, from 4:00-8:00 p.m. in the school cafeteria.

 

What is the deadline to have the residency documentation submitted?

 

Although the process can begin on Monday, April 1, 2019, the deadline for residency documentation is May 31, 2019.  After May 31, 2019, failure to submit residency documentation can delay your registration for the 2019-20 school year, and require attending at least one parent/guardian on campus residency/registration session on Wednesday, August 14 or Thursday, August 15, 2019.

 

Are there fees due or collected during the annual residency verification process?

 

No.  Fees for the 2019-20 school year will be collected in July (beginning July 8, 2019).  Information on the fee payment process will be shared with families in June.

 

My family usually qualifies for a fee waiver and/or free lunch, do I need to complete the residency verification process?

 

Yes.  Every family needs to complete the residency verification process on an annual basis.  The residency verification process is independent of the fee waiver and free lunch application process. 

 

 

 

I am the parent/legal guardian but I am residing with another family within the district.  What paperwork is needed?

 

An Affidavit of Residency must be completed and can be found on page 3 of the Residency Verification Form.  You can also obtain a copy at the security desk located inside our main entrance.  All three pages must be completed.  The property owner/lessee on the Affidavit of Residency is required to provide proof of residency and submit copies of the following:  current driver’s license or State ID, mortgage statement, homeowners insurance, or lease, one current utility bill.  The parent/legal guardian on the Affidavit of Residency must submit copies of the following:  current driver’s license or State ID AND copies of two of the following:  W-2, pay stub, bank statement, Public Aid, Medicaid, vehicle insurance, vehicle registration or other acceptable proof of living at the address used on the notarized affidavit.  If you are utilizing the Affidavit of Residency, you also may be subject to random residency checks by our police liaison, at which time you must show proof that the student and legal parent/guardian reside at said address.  No Exceptions!
