Community Service Log

If anyone is interested in having community service hours added to their transcript, here is the Community Service Log  that students can complete. Community service hours are not required for graduation. This is only for students who want the hours included on their transcript.  

Any student in the National Honor Society (NHS) will automatically have the required 25 service hours (per year) added to their transcript.  If you want credit for any additional hours beyond the 25 hours, you must fill out this form, in addition to the form required for NHS.
