To obtain an official transcript, the cost is $3.00. To obtain your transcript please follow the following 1-2-3 step process:
1. Download and complete the Release of Confidential Information form (click here to download PDF)
2. Print and complete the form using the name that appears on your birth certificate or the name used when you attended high school. Please ensure the graduation year is accurate.
3. Mail the completed form along with a check for $3.00 per copy to:
